34 Students of Concern

Student of Concern Form – This form can be found on the right side of the My USU Homepage under “Reporting.” Students, faculty, and staff can fill out this form if they are concerned about a particular student. Student of Concern reports can be filed if an individual witnesses any form of harassment, if an individual notices a change in student demeanor, or if an individual is worried about student well-being. If one of your students is experiencing signs of distress, you may submit a Student of Concern report (https://studentconduct.usu.edu/reporting).  This report allows a trained official at USU to do a wellness check on the student.

Use the student of Concern Report when you believe:

  • The student may be suicidal
  • The student may be a danger to others
  • The student needs non-academic resources like food, finances, healthcare, or mental health services
  • The student has been behaving in an inappropriate, disruptive, or dangerous manner

If you are unsure which form to use, fill out the form you believe best describes your concern. The Office of Academic Belonging & Learning Excellence will forward your report to the appropriate office if it belongs elsewhere.
