33 Student Academic Achievement Alert
This website from the Division of Student Affairs is used by USU faculty, advisors, and staff to report student academic performance and engagement concerns. If reasonable efforts to resolve issues and provide outreach to students have failed, the Student Academic Achievement Alert form is the next step to provide support. The Office of Academic Belonging & Learning Excellence will oversee this form and work with students to broker success. You will use this form when a student is struggling with academic belonging and/or executive function.
Academic Belonging:
- Student feels incapable or unable to succeed academically (low test grades, etc.)
- Student doubts they have selected the right program of study
- Student believes they do not belong to the community of learners
- Student has a mismatch between aspirations and effort/aptitude
Executive Function:
- Time management
- Study habits and strategies (note taking, etc.)
- Ability to focus attention
- Ability to remember instruction, prioritize tasks, and/or plan
