2. Family Medicine: Back Pain

Evaluation Dialogue Between Doctor-Patient:
Doctor How are you feeling today?
Patient Me duele mucho la espalda. No me puedo mover.
Doctor What is the pain like? Tell me about it. Does it extend towards one side? Does it extend towards your leg?
Patient Sí, me baja por la pierna derecha hasta el pie.
Doctor Does it change depending on your posture?
Patient Sí, no me puedo agachar. Cuando estoy tumbada,  me cuesta mucho levantarme de la cama. Estoy fatal.
Doctor OK, let’s examine you and see what we can do.
Doctor Stand up and lift up your shirt. I’m going to look at your back.
Doctor Now I need you to try to touch the tip of your toes. Afterward, lean back.
Doctor Now we’re going to rotate at your midline. Lie down on the table and relax your legs. Lastly, I need you to walk on your tip toes and on your heels.
Doctor From the exam and from what you’ve told me, it seems that you have sciatic nerve pain.
Patient Vale, pero deme algo porque yo ya no puedo más. He probado de todo y no se pasa.
Doctor Alright, we’ll try an anti-inflammatory such as 600 mg Ibuprofen every eight hours. For the neuropathic pain in your leg, let’s try 75 mg of Pregabalin at night.
Patient Gracias.
Doctor See you. I hope you feel better soon.
