17. Dermatology: Eczema

Evaluation Dialogue Between Doctor-Patient:
Doctor What’s your name? How old are you?
Patient Me llamo Pedro y tengo siete años.
Doctor Tell me what’s happening with your skin.
Patient Tengo rojeces en los brazos y en las piernas. Me pica mucho. Por las noches no puedo dormir del picor.
Doctor How long has this been happening?
Patient Desde hace tres semanas, pero me pasa todos los inviernos.
Doctor Has your son been treated?
Mother Le pongo mucha crema hidratante, pero no mejora.
Doctor Does he have any illnesses? Any allergies?
Mother Tiene asma y alergia al polen de gramíneas y ácaros.
Doctor Any family history of skin illness?
Mother Yo tengo dermatitis atópica.
Doctor Well, Pedro, let’s take off your clothes so I can see your skin.
Post-Examination Dialogue Between Doctor-Patient:
Doctor Pedro has eczema in the creases of his arms and legs. Those red, peeling spots that are so itchy are similar to atopic dermatitis.
Mother ¡Lo sabía!
Doctor When it’s eczema, putting on lotion is not enough, you have to apply treatment creams. In this case, topical corticoids. So, Pedro, I’m going to write you a prescription for a cream that you need to apply thinly over the affected zones once a day, until the eczema is gone, in approximately 7 days.
Patient ¿Y esto me quitará el picor?
Doctor In addition, you’ll take an antihistamine to relieve the itch. You’ll take one pill before bed, so you can sleep, and not scratch yourself during the night.
