Buenos días, doctor. Vengo porque desde hace aproximadamente una semana me cuesta respirar. He ido al médico de cabecera y me ha recomendado venir a visitarlo. |

OK. How old are you? Do you have any important medical history or are you currently receiving any treatments? |
| Patient |
Tengo 86 años. Pues únicamente soy hipertensa, tomo una pastilla de lisinopril por las noches, pero sinceramente no llevo control de las cifras de tensión en mi domicilio desde hace tiempo. |
| Doctor |
So, tell me, when do you notice you’re short of breath? |
| Patient |
Pues inicialmente comenzó cuando realizaba esfuerzos físicos importantes. Pero poco a poco ha ido aumentando en intensidad y sucediendo con menores esfuerzos. Esta noche me costaba respirar incluso en reposo, por eso he venido. |
| Doctor |
Have you noticed anything else apart from shortness of breath lately? Have you gained weight? |
| Patient |
Ahora que lo comenta, he ganado peso. Sobre todo, se me han hinchado el abdomen y las piernas. |

Have you had chest pain or heart palpitations? |
| Patient |
No he tenido dolor. Tampoco tengo sensación de palpitaciones, pero en el electrocardiograma que han realizado en el Centro de Salud, me han dicho que estaba taquicárdica en reposo. |

OK. I’m going to order an echocardiogram and blood work to complete the study of your heart. In the electrocardiogram, you can see arrhythmia frequently associated with aging of the heart, called auricular fibrillation. |

¿Y qué debo hacer doctor? |

We’re going to start a treatment with three medications: a diuretic, another to control your heartbeat, and an anticoagulant. |

We’ll review the results of the tests when you come in for a consultation, and see if it’s necessary to do any other tests. If you notice your shortness of breath worsens or doesn’t go away, go to the emergency room. |
