Appendix K EBP Project Peer Evaluation and Contributions

Peer Evaluation and Contributions Form

Directions: Evaluation your peers below. Be sure to include their name, answering each question with rationales/explanations, and assign a contribution score. A score of zero would be equivalent with never showing up, contributing nothing, and not being present. A score of 10 would mean the individual contributed what would be reasonable and expected for any student to contribute.

Explanation of how grading works:

You will receive full points if you comprehensively complete this form. Instructor (and mentors, if needed), will review the evaluations of your group members. If, for example, a group member receives scores of 9/10, 10/10, 6/10, 9/10, and 10/10, the average will be taken from these scores (in this case, it would be 8.8). This means that the member’s group felt the individual group member only contributed 88% (and not 100%) and would likely receive 88% of the total grade EBP Project. If the project, for example, received 100%, the individual that only contributed 8.8 would receive 88% while the others may receive 100%. However, I will likely drop any low outliers, so would throw out the one 6/10 score. This helps to prevent individual bias toward a group member.

Further, if an individual deems him/herself on the self-evaluation as having contributed fairly and the majority of group members scored the individual as having contributed far below equity, this will be taken into consideration and a final grade will be awarded with this in mind. Be sure you are honest and transparent with your self-evaluations.
